We are conducting a survey among people who submitted an application to UDI in 2024. The survey consists of a questionnaire, and some applicants are randomly selected to participate.
If you are selected, you will receive the questionnaire by email. Participation is voluntary, and your responses will remain anonymous. UDI will not know who has responded.
By answering the questionnaire, you can share your opinion about your experience with UDI. Your feedback helps us understand what we can make easier and better.

- Do not include personal information or details about your case in the questionnaire.
